Page 2 of 7 Public Works June 15, 2011 Resolved that Report DPW11-22 dated May 24, 2011 regarding Mary Drive Dedication as Public Highway be received and the Mayor and Town Clerk be authorized to sign a By-law dedicating the following as Municipal Public Highway known as Mary Drive: i) Plan 30R-13558, Part 1 and Part 2; and, ii) Lot 26 (1-foot reserve) on Registered Plan 506, and further that the By-law be registered under The Land Titles and Registry Office.

Resolved that Report DPW11-23, dated May 30, 2011, regarding 381 Murray Street, Grimsby, Draft Plan of Subdivision - Public Works Draft Plan Comments be received; and that the recommendations and the Twenty Six (26) Draft Plan Conditions therein contained be approved; and further, that the Draft Plan Conditions be included in 381 Murray Street, Grimsby, Draft Plan of Subdivision - Conditions of Final Approval.

Page 3 of 7 Public Works June 15, 2011 Resolved that Report DPW11-24 dated June 3, 2011 regarding „Drinking Water Quality Management System (DWQMS) Operational Plan Re- endorsement‟ be received by Town Council for review and that this report and the following recommendations be approved:  that Council endorse the current revision of the DWQMS Operational Plan, included with this report as Appendix „A‟; and  that Council re-endorse the DWQMS Policy statement, included with this report as Appendix „B‟; and  that the Mayor be authorized to execute Section 3.0 of the DWQMS Operational Plan endorsing the current revision of the document; and Enclosed Documents The following documents have been included with this report: 1. Town of Grimsby DWQMS Operational Plan for the Grimsby Water Distribution System, Revision 5, June 2011, included as Appendix „A‟; 2. Town of Grimsby DWQMS Policy statement, Revision 2, May 2010, included as Appendix „B‟; 3. Operational Plan - Summary of Interim Revisions, Revision 5, June 2011, included as Appendix „C‟.

Page 4 of 7 Public Works June 15, 2011 Resolved that Report DPW11-25 dated June 6, 2011 regarding Engineering Services for Watermain Project PWC12-06-440028 Robinson Street South, Lake Street, Park Road North, Marsdale Avenue, Kennedy Road, Nassau Drive be received and that the Director of Public Works be authorized to award the design, contract administration, and on-site supervision to Quartek Group Inc. in the total estimated amount of $76,110.00.

Resolved that Report DPW11-26, dated June 7, 2011, regarding Award of Contract PWC11-52130-C, Supply of One (1) New 2011 Full-Size Standard Cab Pickup Truck (6,000 lbs. GVWR) be received; and further, that the quote from Terrace Ford Lincoln Sales Ltd. (Burlington, ON) in the amount of $25,047.58 ($22,166.00 plus HST) for one (1) full- size standard cab pickup truck be accepted.

Resolved that Report DPW11-27, dated June 8, 2011, regarding amendments to Schedule “A” of By-law 73-41 for fees for installation of water service pipes, on and off charges and collection on account charges Page 5 of 7 Public Works June 15, 2011 and Schedule “B” of By-law 73-41 for new water meters and remote readers, water meter rebates and water meter testing, be approved and further that the Mayor and Town Clerk be authourized to execute the attached amending By-law No. 11-38.

Resolved that approval will be granted for the Festival at the Forty scheduled Saturday, August 13, 2011 to be held subject to Christie Street (bridge) being open to traffic on the day of the event. Should Christie Street be closed, the event will be limited to Main Street (Reg. Rd 81) between Christie Street and the west side of Ontario Street road allowance.

Resolved that correspondence from Grimsby Power dated May 16, 2011 regarding the cancellation of streetlight services be received for information.

Resolved that correspondence of May 30, 2011 from the Lakeview (Child's View Co-operative) - 381 Murray Street requesting a sidewalk on the east side of Murray Street and along the south side of Olive Street for access for the Day Care children to Lakeview School be received and that Murray Street section ($8,000) be considered within the 2012 Capital Budget as a component of a new south side Olive Street sidewalk running from Lady Fatima to Lakeview School ($55,000).

Resolved that the minutes of the DIA meeting held April 21, 2011 be received for information and filed.
